If you need to update a provider profile, follow these steps instead of submitting this form:

• Profile photo updates – email AAH-ProviderPublicAffairs@aah.org.
• Illinois profile changes – Make changes at this website. Quick tip: You can also request edits by clicking “Is this your profile? Submit edits/updates” at the bottom of a profile page.
• Wisconsin profile changes – Sign into Caregiver Connect and visit the Provider & Location Data Management (PLDM) intranet site.

• Illinois locations – Click “Edit practice page” in the upper right corner of the location page you want to edit and fill out the form fields.
• Wisconsin locations: Email changes to dataintegrity@aah.org.
