Choose this option to make a request on pages that are accessible to the public. These include sites like advocatehealth.com or aurorahealthcare.org.

Choose this option if you are having a technical issue with a Caregiver Connect page that is not accessible to the public. This includes sites created for WI-based team members, business applications, HR systems and custom web applications – this includes most digital property accessed through Caregiver Connect.

To make a request on a SharePoint department page, you must contact the individual site owner. Please refer to the department's website for contact information.
